Faculty Activity Report
Each faculty member is required to submit a Faculty Activity Report each year. Click here to see a blank template for the 2006 Faculty Activity Report Form. The form changes slightly from year to year and we hope to have an on line system soon. You will be notified by your department in the Dec./Jan timeframe that you will need to complete the faculty activity report. An up-to-date form and a due date will be provided at that time.
Take a look at the form and set up a filing system which reminds you to save all pertinent information in an orderly, easy to retrieve format.
